Completing the Annual Statement of Commitment
Each spring, HLC peer reviewers and Institutional Actions Council members submit their Annual Statement of Commitment and Conflict of Interest Disclosure in Canopy.
Follow these steps to successfully submit this form:
- After logging into Canopy, the Annual Statement of Commitment form will be listed under My Tasks on the peer reviewer dashboard. Click the title to open the form.
- On the form landing page, click “2024 Annual Statement of Commitment and Conflict of Interest Disclosure” in the left navigation bar or in the main body of the page to open this section of the form.
- Review the instructions for this section, and then scroll down to the box titled Annual Statement of Commitment. Click the “Edit” link on the right-hand side of the box.
- Complete all required fields in the form:
- Check the first two boxes to confirm that you will abide by HLC’s policies regarding objectivity, conflicts of interest, and standards of conduct for peer reviewers.
- IAC Public Members: Check the third checkbox on the page to affirm that you remain eligible to serve as a public member. (All other peer reviewers should skip this question.)
- Enter your full name in the text field.
- Click the Review and Submit button to save your response and go to the final page of the form.
- At the bottom of the Review and Submit page, check the box and click the Submit button.
- In the dialog box that appears, click Submit again. You will see a message confirming the form was sent to HLC.
- If you return to the peer reviewer dashboard, the form will now be listed under the “Completed” tab of the My Tasks section.
