FSA Status Notification

HLC member institutions should complete the form below to notify HLC if they are placed on, or removed from, any of the following statuses by the U.S. Department of Education office of Federal Student Aid:

Note: Per HLC policy, institutions under these statuses are subject to additional requirements:

  • Provisional Certification. An institution under a provisional certification status that was assigned on or after July 1, 2020, (1) is subject to additional requirements related to substantive change and (2) is not eligible for HLC’s Notification Program for Additional Locations.
  • Heightened Cash Monitoring 2 and Reimbursement Payment Methods. Institutions placed on either of these payment methods are required to submit a Provisional Plan to HLC for approval.

This form should only be completed by an institution’s Chief Executive Officer, Accreditation Liaison Officer or Data Update Coordinator. 

Submit the form separately for each status change the institution needs to report.
