To keep HLC informed of important developments that may impact students, member institutions are required to notify us when certain actions are taken by the U.S. Department of Education, state agencies or other recognized accreditors.

Actions to Report
The institution being placed on or removed from any of the following:
- Provisional certification for participation in Title IV.
- Heightened Cash Monitoring 2 payment method for financial aid disbursements.
- Reimbursement payment method for financial aid disbursements.

State Agency Actions
Actions to Report
A state agency issuing a pending or final action that affects the institution’s legal status or authority to grant degrees in the state.

Actions by Other Recognized Accreditors
Actions to Report
- The institution receiving a pending or final adverse action from a recognized accreditor.
- The institution being placed on sanction by a recognized accreditor.
How to Notify HLC
Email [email protected]. In the email, include a copy of the action letter from the accreditor.
