Authorized representatives from HLC-accredited institutions should complete this form when the institution receives a request from a regulatory agency, such as a state agency or the U.S. Department of Education, to:

  • Verify the institution’s accredited status.
  • Document that HLC approved a particular program or location.
  • Provide other information confirming an aspect(s) of the institution’s scope of accreditation by HLC.

For example, this form should be used when an institution needs to provide documentation for certification in the Student and Exchange Visitor Information System (SEVIS) with the U.S. Department of Homeland Security.

Process and Timeline for Letter Requests

After completing the form, you will receive an email confirming that HLC has received your request. HLC staff will review the request against the institution’s file and relevant HLC policy, and will send a letter to the individual or entity identified in the form. Staff will contact you if any additional information is required. Depending on the complexity of the request, this process may take 1–2 weeks.
