Institutional Records Request
An institution’s Accreditation Liaison Officer (ALO) or chief executive officer (CEO) may request copies of certain documents related to the institution’s accreditation from HLC at any time. Complete the form below to submit this request.
HLC provides this service to assist institutions in updating any missing files with the expectation that institutions develop their own process for tracking and maintaining accreditation-related files.
Fee
The fee for this service can be found on HLC’s Dues and Fees Schedule.
Records Provided
HLC will provide the records listed below for the past 10 years. The scope of the request cannot be changed to cover a larger or smaller scope of materials or to cover a different time frame.
- Narrative and related documentary materials submitted by the institution as part of an evaluation process. This could include, for example, the Institutional Update, a substantive change application, an Assurance Argument or an institutional response.
- Materials prepared by HLC regarding the institution as part of an evaluation process. This could include, for example, a final team report, an Institutional Actions Council report or a desk review.
- Action letters and other official correspondence related to evaluation activity. This could include, for example, an action letter indicating action taken by the HLC Board of Trustees, an action letter indicating action taken by the Institutional Actions Council, or a letter imposing an institutional designation.
Process and Timeline
After completing the form, you will receive an email confirming that HLC has received your request. HLC will send the files to you through a secure file transfer link. This process may take 1–2 weeks.
